The following form is to show your interest in enrolling your child. If you meet our requirements, more information will be required including the following: 

  • Shot Records
  • Physician’s Statement of Health
  • Vision and Hearing Screening
  • Child's Birth Certificate
  • Driver’s License of Custodial Parent or Legal Guardian

You may also be required to supply the following:

  • 2 Pay Stubs for Parent or Legal Guardian
  • 2 Pay Stubs for Additional Parent or Legal Guardian
  • Tax Return
  • Tax Return for Additional Parent or Legal Guardian
  • Letter From Employer
  • Letter From Employer for Additional Parent or Legal Guardian
  • Current Class Schedule (Print schedule from institution registration information)
  • Proof of Enrollment (Letter/email that indicates enrollment dates)
  • Unofficial Transcript or Degree Audit (Print from online or contact your registrar office)
  • Proof of Legal Guardianship (Court documents indicating guardianship rights)
